Job Description:

The Registered Dietitian CDE is responsible for overseeing the planning, implementation, and evaluation of the Diabetes Self-Management Education (DSME) program at primary care locations. The Registered Dietitian CDE will work with RNs and providers to provide a multi-disciplinary approach to DSME in order to bring all facets of DSME to the patient population. Ensures that the National Standards (NSDSME) are always met and maintained. Acts as the key resource on nutrition and dietetic related information for staff. The Registered Dietitian CDE is part of the person-centered integrated care team and provides Medical Nutrition Therapy (MNT) and DSME for clients enrolled in the Primary Care Services. ESSENTIAL FUNCTIONS JOB SPECIFIC Diabetes Program Coordinator Plan, implement, and evaluate the DSME program with collaboration from a multi-disciplinary team Arrange and coordinate the activities of the DSME Advisory Committee Liaison between staff, Advisory Committee, department heads, and administration Responsible for obtaining and maintaining ADA Recognition and participating in the evaluation of the programs effectiveness Maintain qualifications (CE credits, practice hours, licensure, and registration) Review dietary services being offered at all Compass Network locations, as requested, to assist in promoting common standards across departments Certified Diabetes Educator and Registered Dietitian Nutritionist Provide individual and group diabetes self-management education/training (DSME/T) to patients (and family members) to enable them to learn techniques to bring their disease under control, avoid potential complications and bring about lasting improvements in quality of life Provide Medical Nutrition Therapy (MNT) to patients (and family) to assist in the treatment of patients chronic diseases Utilize telehealth technology to serve clients restricted by location and transportation Provide nutrition counseling for patients suffering from an Eating Disorder in conjunction with the therapy department Provide individual and group/team staff consultation on specific clients Train and provide nutrition and diabetes education to patients support staff Utilize Motivational Interviewing and/or other techniques that assist with health behavior change Other Responsibilities Attend all training as required by Compass Health Network, Accreditors and Funders Regular attendance except as excused Other duties as assigned ESSENTIAL FUNCTIONS/CORE COMPETENCIES COMPANY WIDE Regularly attends work except for approved absences Attends all training as required Maintain professional, champion behavior Provides quality customer service Provides services with a commitment to service excellence Works in a way that exhibits a commitment to personal excellence Team oriented and values diversity DESIRED DEMONSTRATED KEY COMPETENCIES Analytical Thinking Collaborative Relationships Creative Thinking Communication Oral Communication Written Decision Making Empowers Others Flexible Thinking Leadership Strategic Thinking Teamwork Technical Expertise EDUCATION Bachelor's degree required Master's degree preferred WORK EXPERIENCE BLS training required (If not obtained upon hire, Compass Health provides this to staff whose positions required it) Dietary knowledge as it specifically relates to the diagnoses of patients (i.e. chronic medical conditions and various behavioral health conditions in adults and children) required Working knowledge of budget conscious healthy eating plans (i.e. for people who have a limited food budget, those that depend on food stamps, food pantries, etc.) required Working knowledge of techniques for affecting health behavioral change for clients with a Serious Mental Illness required LICENSURE/CERTIFICATION Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration (CDR) Certified Diabetes Educator (CDE) by the National Certification Board of Diabetic Educators (NCBDE) Compass Health is an Equal Opportunity/Affirmative Action Employer and an E-Verify participant.
