Outpatient - Preferred Certified Diabetes Educators - 40-Hours per
week - Day Shift - Benefit Eligible - NO CALL, NO WEEKENDS, NO

Qualifications Role Purpose Provides Diabetes and Nutritional
educational services and community resources for inpatients and
outpatients. Maintains a case load of patients and promotes program
development. Responsibilities - Monitors patients response and
modifies plan of care accordingly. - Provides accurate
documentation as required. - Delivers direct patient care by
assessing and evaluating patient's physical, psychosocial and
emotional needs. - Develops and implements individual plans of care
in accordance with the established guidelines and standards of
nursing care. - Educates patients and their families on how to
manage their condition. Minimum Requirements Education - Nursing
Diploma/Associate's - Nursing/Nutrition/related Experience -
Supervisor Experience - No Experience Licenses & Certifications -
RN Preferred Requirements Education - Bachelor's Degree - Nursing
