We are currently seeking a compassionate and skilled RN-Nurse Navigator to join our dedicated team. In this vital role, you'll act as a liaison between patients and healthcare providers, ensuring seamless coordination of services across our extensive healthcare network. Your expertise will guide patients through complex healthcare environments, facilitating access to necessary medical care and improving overall clinical outcomes. Coordinate effectively between patients and medical teams to ensure clear communication and proper treatment plans Educate patients and their families on diagnoses, treatment options, and the healthcare resources available Monitor patient progress and advocate for timely interventions and services Facilitate the appropriate use of healthcare services, enhancing efficiency and patient satisfaction Maintain comprehensive documentation of patients' care plans and interventions Participate in multidisciplinary teams to improve patient care processes and outcomes Support patients in understanding their rights and responsibilities within the healthcare system Strong communication and interpersonal skills Excellent organizational and problem-solving abilities Ability to work collaboratively in a multidisciplinary team Proficient in healthcare IT systems Empathy and dedication to patient care Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license BLS Certification Certification in Case Management (CCM) preferred but not required
