AvailableSUMMARY: Lead Staff are continually involved in the
assessment, development, and/or management of an individual's
Support Plan. Lead Staff support individuals by coordinating
services and facilitating opportunities that include options,
choices, independence, empowerment, and community inclusion. Lead
Staff have a demonstrated commitment to the vision, mission,
values, goals and objectives of the Agency.ESSENTIAL DUTIES AND
RESPONSIBILITES:This list of essential functions is not intended to

REQUIREMENTS:A. Provide care to people we support in accordance
with the Individualized Support Plan [ISP's], Behavior Support
Plans [BSP's], staffing patterns, and budgetsB. Follow employee
schedules, as assigned, in accordance with staffing patterns and
budgets (i.e. overtime, accuracy, and completeness)C. Assist Home
Manager with developing and maintaining staff schedule according to
staffing patternD. Must work 40 hours per week, and shifts will
typically be opposite of the Home Manager. Must be available to
provide temporary program coverage, as necessary, to ensure the
safety of the individuals served (weekend call-offs, emergency
supervision needs, etc.)E. Check and read Sunnyhill emails when
working at the home and respond in a timely manner to provide
follow-up as appropriate.F. Attend and participate in professional
training at the Sunnyhill office as assigned by supervisors. Attend
in-home team meetings on site.G. Communicate personal training
needs and provide written requests for training to the assigned
Home Manager and/or Program Supervisor.H. Communicate positively
and effectively with people we support, Guardian's, Case Managers,
and other treatment providers in the communityI. Work with team
members to complete personal outcomes. Complete accurate and
relevant log notes that addresses individual personal outcomesJ.
Ensure the safety of all individuals receiving services is
maintained by ensuring that all staff working at the site complete
initial emergency drills, monthly fire drills and quarterly
earthquake, tornado and intruder drills. Will verify that all
drills are submitted by the 5th of the following month.K. Verify
census compliance in taskmasterL. Provide personal assistance in
hygiene, toileting, bathing, eating and other activities of daily
living as each individual needs it or as outlined in the individual
ISPsM. Review and sign all individual ISPs, BSPs, Monthly Nurse
Summaries, and AddendumsN. Adhere to all paperwork deadlines, as
stipulated by supervisorsO. Utilize and enforce the use of the
Chain of Command for the site and act as first person in chain of
command every other weekend and when the Home Manager is on PTO.P.
When performing on call duties, will respond within one hour to
phone calls in support of assigned work siteQ. Follow cleaning and
maintenance schedule to ensure home is well-keptR. Will assist Home
Manager with managing timely/accurate financial logs, bank
statements, receipts, and check registers by organizing all
receipts from purchases. Ensure that funds/financial documentation
remain secured safely in the home.S. Obtain approval for and
schedule all outings in advance and document accordinglyT.
Administer all medications and medical treatments accurately and
document accordingly. Ensure all prescribed medications and
treatments are available and taken as directed by maintaining
current orders and supplies. Review Medication Administration
Records (MARS) once weekly.U. Monitor, document, and adhere to the
nutritional/dietary needs of consumersV. Ensures individual home is
supplied with adequate amount of groceriesW. Ensure that all
individuals have the appropriate household and adaptive equipment
available for use at all times and are in good working orderX.
Ensure that requests, for household and adaptive equipment and/or
repair, are communicated to a supervisor (within 24 hours of
noticing the need)Y. Transport individuals for home shopping and
other outings in personal and/or company vehicle as
required/assigned by a supervisor (This may include obtaining a
chauffeur's license)Z. Communicate with the assigned supervisor
and/or agency nurse (immediately) when any unusual medical or
behavioral incidents occurs during the care of an individualAA.
Attends individual doctors' appointments during shifts including
Emergency appointments, Urgent care/ER visits. Preferably without
overtime. Will change schedule to adjust for hours worked, if in
overtime.BB. To be trained on within 90 days: Schedule for the
home, financial ledgers, Monthly Outcome Reports (MORs), updating
the ISPCC. Ensure that all individuals are free from abuse and
neglect in the home and when out in the community and immediately
report the suspicion of abuse and neglect to the assigned
supervisorDD. Represent Sunnyhill in a professional manner at all
times including but not limited to acting as a role model for
employees regarding individual care, financial procedures, etc.EE.
Satisfactorily completes all other duties and special projects as
assigned by supervisorThe above statement reflects the general

Human Service field experience preferred2. Other requirements: Use
of a personal motor vehicle, valid driver's license, adequate auto
insurance as defined by state law. Acceptable police, DFS, and DMV
record checks. Must successfully complete all company provided
training.WORKING ENVIRONMENT:This position could work in any of the
following environments;1. Office2. Clients' home3. Medical Office
buildings4. Other settings within the community5. May be required
to travelProtective Equipment Required: As required by task and
procedure.Environmental Exposures: May be required to walk outdoors
in all seasons. May be exposed to occasional loud noises.PHYSICAL
REQUIRMENTS:1. Manual dexterity skills, including repetitive use of
hands, fingers (and grasping)2. Able to stand, walk, and sit for up
to four (4) hours at a time.3. Repetitive bending and reaching up
to two (2) hours a day.4. Average vision/hearing.5. Able to lift 25
pounds from floor to waist.6. Able to navigate stairs.Non profit,
